The $1.2B Administrative Storm Threatening Maryland’s Safety Net
As Open Enrollment begins and strict Medicaid redeterminations loom for 320,000 Marylanders, community navigators are caught between rigid state mandates and rising hospital uncompensated care. Here is who wins, who loses, and why Annapolis will bear the bill.

On November 1, Open Enrollment officially opens across the Maryland Health Connection. In any normal year, state health officials and community outreach networks would celebrate the seasonal push to drive down Maryland’s uninsured rate.
This year, no one in the safety-net space is celebrating.
Behind the press releases and public awareness campaigns lies an administrative reckoning scheduled for January 1. For the estimated 300,000 to 320,000 low-income Marylanders enrolled under ACA Medicaid expansion (ages 19–64), the rules of coverage are undergoing a seismic shift.
Federal and state mandates are enforcing an 80-hour-per-month work, education, or volunteer requirement, paired with a shift from annual renewals to six-month redetermination cycles.

The frontline burden of this transition doesn’t fall on government agencies. It falls directly on Maryland’s safety-net navigation entities—nonprofit health networks, Federally Qualified Health Centers (FQHCs), and community outreach organizations.
These organizations are built to enroll people into healthcare. Starting January 1, they are being drafted to act as compliance auditors, activity verification centers, and paperwork trackers.
